The ADVANTAGE HEALTH plan was designed for beneficiaries that have been diagnosed with certain chronic conditions such as diabetes, cardiovascular disorders or chronic heart failure. This plan includes all the QHP extra benefits such as the part D prescription drug coverage, fitness center membership, dental, hearing, vision, etc. See plan materials for details.

The VALUE ONE plan offer recipients who qualify for BOTH MEDICARE AND MEDICAID an opportunity to receive benefits designed to offer more than the current coverage. QHP can also assist you in determining Medicaid qualification. VALUE ONE eligible beneficiaries can make changes to their coverage at any time during the year.

This plan is available to those diagnosed with certain chronic aliments such as Cardiovascular disorders, Chronic heart failure or Diabetes Mellitus.  If you have one of these illnesses please contact us to find out more.  This plan is available in all Richmond, Queens and Nassau counties. You may join the plan at anytime.  Benefits are generous with low co-pays.  Advantage Health NY includes Health/Fitness club membership, Dental, Vision and Hearing benefits as well as a quarterly Over-The-Counter Drug and Supply allowance.  Part D Drug benefits come with this plan.

This plan is available to those eligible for both Medicare and Medicaid. This is probably the best way to reduce your medical expenses but many will rush to the speedy conclusion that they are not eligible.  Don't rush to judgment.  Let us help you find out for sure if you or a loved one are eligible for Advantage Value One NY.  This plan is available in all 4 New York counties served and is open for enrollments all year around...there is no "Lock-In".  Depending on your level of Medicaid eligibility, many co-pays are $0, including hospital stays, tier 1 drugs, screenings and immunizations as well as selected hearing, dental and vision services.  Like other QHP plans, Advantage Value One NY offers Health/Fitness club membership.
