Service Area: Quality
Health Plans currently provides Medicare Part D coverage in
various Florida counties. Our Stand-alone Prescription
Drug Plans cover the whole state of Florida. Our
Medicare health plans are offered in the following
counties: Brevard, Broward, Charlotte, Citrus, Clay, Duval, Hernando, Hillsborough,
Indian River, Lake, Lee, Manatee, Marion, Martin, Maimi-Dade, Okeechobee, Orange, Osceola,
Palm Beach, Pasco, Pinellas, Polk, Sarasota, St. Johns, St. Lucie,
Seminole, Sumter and Volusia. Not all plans cover all
of these counties.
Individual Plans Include:
1- Quality Rx: Covers the entire state of
Florida with a monthly premium of $20.10
2- Quality Rx Plus: Covers the entire
state of Florida with a monthly premium of $36.70
3- Advantage:
offered in
Brevard, Charlotte, Citrus, Clay, Duval, Hernando, Hillsborough,
Indian River, Lake, Lee, Manatee, Marion, Martin, Okeechobee, Orange, Osceola,
Pasco, Pinellas, Polk, Sarasota, St. Johns, St. Lucie,
Seminole, Sumter and Volusia. There is no premium;
rather the member is reimbursed up to $51.00 per month
4- Advantage Silver:
offered in
Brevard, Charlotte, Citrus, Hernando, Hillsborough,
Indian River, Lee, Manatee, Martin, Okeechobee, Orange, Osceola, Pasco, Pinellas,
Polk, Sarasota, St. Lucie, Seminole and Volusia with no

These plans provide
out-patient drug coverage through a
broad network of preferred pharmacies. Covered drugs are
those included in the plan formulary which may be viewed
on-line, either in abridged version or the complete
formulary. The abridged version also has important
information on how your co-pay is determined, restrictions
on coverage, how to request an exception to coverage rules
and other information pertaining to all plans.
Quality Health
Plans is required to give 60-day notice regarding removal or
change in the preferred or tiered cost-sharing status of a
Part D drug. This information is to be maintained on the Web
site until the next annual mailing of the updated formulary.

such as co-payments and co-insurance vary from plan to
plan. Please see your particular plan’s summary of benefits
to determine your cost sharing parameters.
